I create a record of the care and services you receive from me in order to provide you with quality care and to comply with certain legal requirements. Federal laws (HIPPA, HITECH) require all medical records and other individually identifiable health information used or disclosed by me in any form, whether electronic, on paper, or orally, to be kept properly confidential. I utilize an electronic records management system that is fully compliant with HIPAA and HITECH standards for encrypted transmission and storage of protected health information (PHI). I will only release your PHI in accordance with state and federal laws and the ethics of my profession. As per Texas law, I will keep client records for five years for adult clients.
Children's Privacy
If you are under 17 years of age, you may browse the Site. However, you may not provide personal information to us. This Site is not directed to children under the age of 17 and we do not knowingly collect personal information from children under the age of 17 on the Site. If we become aware that we have inadvertently received personal information from a visitor under the age of 17 on the Site, we will delete the information from our records.
Compliance with Law & Fraud Protection
We may disclose any information we deem necessary, in our sole discretion, to comply with any applicable law, regulation, legal process or governmental request. In addition, we may disclose any information when we believe, in our sole discretion, disclosure is necessary or appropriate to prevent physical harm or financial loss or in connection with suspected or actual illegal activity. We adhere to HIPAA laws, state and federal confidentiality laws, and the ethics of professional organizations to which the counselor belongs.
